This Model of Care has been developed in partnership with HSE Social Inclusion, HSE Health and Wellbeing and the National Office of Suicide Prevention and in collaboration with voluntary bodies including Sec.39 agencies. It caters to the needs of Adolescents and Adults through separate clinical pathways - mental health and substance use disorder. The planned service delivery through separate Dual Diagnosis teams for Adolescents and Adults will help deliver services in a more targeted and age appropriate manner.
